Data collection at the Patient Interface

The real strength of the Organiser lies in its flexibility to hold any number of records or documents and its location at the point of administration of medication to the patient. Adapting record cards to accept Bar Codes allows data to be collected on the cards by Health Professionals wherever the patient may be located at that time.

The Pharmacy is well placed to evaluate and transmit urgent and sensitive messages to the relevant authority in a digital format and attract Funding from bodies eager for data relating to Patient Health, Wellbeing and ADRs. The significance of this is that Health Professionals are not only able to access records immediately but are also able to feed crucial information into a database in digital format.

The following are by no means the limits of the possibilities and the needs of the recipients but they may serve to stimulate their imagination.

Adverse Drug Reactions (ADR)
Wellbeing & Compliance

ADRs can be captured by Carers as they occur on integral MAR cards within the unit. The appropriate Bar Code on the MAR may be MANUALLY ticked by the Carer and after verification scanned by the pharmacist.

The Drug would be scanned FIRST, then the symptom and its severity prior to transmission to the Fund Holder.

It is possible that the combination of drugs that a specific patient is taking could also be transmitted which might highlight ADRs across the group not hitherto known.

Who is better placed than the Carer to observe and report potential tragedies at the point of contact with the patient?

The advantage of the combined medication with records unit is portability. The unit may therefore accompany the patient to Hospital, Day Centre, GP surgery or some other Health worker and at all these locations an incident or decision may need to be communicated.

To have a system in place in the Community which can report quickly in digital format to the pharmacy for onward transmission to the Accountable authority is beyond value.
